Creating a Survey
You can create various types of surveys, such as those regarding students' PC and internet access environments, or their level of understanding of course content.
Anonymous surveys can now also be created.
Steps for Creation

Click ⊕ in the Course Content area and select [Create] from the menu.

A panel will appear on the right. Under Assessment, select [Form].

The form creation page will open. Enter the form name in the text box at the top of the page.

Click ⊕ and select a question type from the menu that appears.

Click the gear icon to change the form settings.

Once settings are complete, click [Save].
Question Types
There are four types of question formats available in forms. You can combine these formats when creating a survey.

| 1 | Essay question |
A format where respondents enter answers into a textbox. |
| 2 | Likert scale |
A format using multi-level scales to respond to a topic. Options include 3-point, 5-point, or 7-point scales. |
| 3 | Multiple Choice |
A format where one or more correct answers can be selected from multiple choices. |
| 4 | True/False |
A format where the respondent selects "True" or "False" for a statement. |
To create a binary question with options other than "True/False", you can use a multiple choice question with two options.
Survey Settings

Due Date: Set a due date. If no due date is needed, check [No Due Date].

Additional settings as needed:

Disallow late submissions
-
Prevent new submissions after the due date

Allow class conversations
-
※ Enabling this allows users to exchange text messages within the submission screen. (This is not real-time chat.)

Form Display Options (as needed):

Display one question at a time
-
Randomize question order
-
Randomize answer choices
-
Randomize page order
-
※ Effective only if page breaks have been added.

Number of Allowed Attempts: Choose between 1–10 or Unlimited.

Anonymous Submission: To make the survey anonymous, check [Anonymous Submission].
Once all settings are configured, click [Save].
Completion
To publish the survey to students, change the visibility to [Visible to Students].
※ To check how the form will appear to students, click [Student Preview] in the top-right of the page.
How to Review Survey Responses
You can check submitted survey responses either on-screen or by downloading them as a CSV file.
Viewing Method

Click the survey listed in Course Content.

On the form settings page, click the [Submissions] tab at the top.

The submission page will open. Click the row of a student whose status is "Submitted".

You can now view the contents of their submission.
Download Method

On the form submission page, click […] on the right side of the title row.

Click [Download All].
-
A panel will appear on the right. Adjust settings as needed and click [Download].

The file will then be downloaded.
